Transportation is not a problem! Living Well is contracted with MTM to provide transportation to and from your residence.
Your insurance covers for you to attend Living Well. We accept Medicaid, United, and Neighborhood.

The admission process is quick and simple. First, we strongly suggest you complete an application prior to touring. You can find a copy of of the application below and either email it to us at livingwelladultdaycareri@gmail.com or bring it with you to your tour.
During the guided tour, potential participants and their guardians or case workers will see a plethora of activities in action, have an opportunity to meet our friendly and knowledgeable staff, and ask our Nurse and Manager any questions they may have.
The minimum required age to attend is 21 years old
