Our Mission

The mission of Living Well Adult Day Program is to provide services to adults with disabling conditions and cognitive disorders in cooperation with their families and other professionals who serve them.   

Living Well will increase the level of independence through the development of individual care plans tailored to participant needs and preferences. The Program is designed to improve the overall health and psychosocial status of each individual through education in self-management and a variety of group and independent activities. 

Living Well will promote autonomy for its participants. They have a right to choose activities geared towards their care plan goals. Individual dignity, safety, and confidentiality are central to the philosophy of our environment.
